Facility Use Application
You may submit an application online or download a fillable/printable application.
There are multiple ways to submit your completed application:
- Mail or bring it to Community Education Facility Use, Room 204, District Education Center, 2520 E 12th Ave, North St. Paul, MN 55109. Our office is open Monday–Friday 8:00 am–4:30 pm. An after-hours drop box is available.
- Fax your application to 651-748-7497.
- Email your application to lnapoli@isd622.org or dwheeler@isd622.org.
If you are unable to print an application, please call 651-748-7439 to have one mailed to you, or you can pick one up in our office.

Internal Facility Use Request (for district employees)
District employees must complete an Internal Facility Use Request.

Concession Stand Agreement
You must complete a concession stand agreement online before operating a concession stand in ISD 622 facilities. The agreement is good for all concession stand use for the period of July 1–June 30 during the year you complete the agreement.

Fundraiser Information Form
Please complete a separate fundraiser information form for each fundraising event. You may list multiple dates and/or locations on the same form if they are for the same fundraising event.

Gym/Field Cancellation Form
Notify us of canceled practices before noon the day of practice by completing this online form. Please contact your organization's designated scheduler if you need to reschedule a canceled practice. -
Weekday Gym Use Survey
Complete the Weekday Gym Use Survey to request gyms for weekday practices/activities from November 1 - March 11. -
Weekend Gym Use Request
We are currently accepting requests for weekend gym use. Requests will be processed beginning July 1 in accordance with our Facility Use Procedures. To submit a request use this online form.
