    In order to limit the number of people in the building at one time, Please sign up for a time slot to attend. . Use one sign-up slot per family; you do not need to register each family member attending individually.
  • Immunization Clinics

     
     
    School District 622, in partnership with Homeland Health, will hold immunization clinics for District 622 students only who are not up to date on their required shots prior to starting school. Clinics will be held in conjunction with the middle school open houses, with two additional clinics offered for all students enrolled in 622 schools at North and Tartan High Schools.
     
    The immunization clinics will be held at: 

    Thursday, August 25 - Tartan High School

    Wednesday, August 31 - John Glenn Middle School

    For more details or to download the required Immunization Form, visit www.isd622.org/immunizations. Immunizations will not be given without this form.
     
    A $20 administrative fee is requested per student, but no one will be turned away for inability to pay. Families with insurance should bring their insurance card with them. Students should wear short sleeves to make it easier to get vaccinated.

  • Application for Educational Benefits

    Every ISD 622 family should complete an Application for Educational Benefits. The application helps us identify families who need assistance, not only with meals, but also may provide families discounted rates on athletics and activities fees, and testing fees, among others. Completed applications also allow District 622 to receive funding for classroom programming. This funding helps us stretch our local dollars further.
     
    Families must complete a new form each school year. Also, families who have completed an application and have experienced a change in household income or size are encouraged to complete a new application.
