Sports Physicals
Free sports physicals will be available on Monday, August 10, and Friday, August 28, from 9 a.m. - 3 p.m. at the District Education Center (DEC) Boardroom.
Sports physicals are free to all students, regardless of insurance coverage.
Bring:
- Medical insurance information
- Consent to Treat form - completed by parent/guardian if they can't attend with you
- Sports physical form - available in English, Hmong, Somali, and Spanish
