Health Partners Free Sport Physicals @ Our Well@Work Clinic

Thursday, August 11th 10:00am-6:00pm

Well@Work Clinic located in the District Education Center.

Please download and print the 2022-23 Sports Physical form HEREand take it with you.

Please download and print the Consent Form HERE and take it with you to your appointment.

Students will need to register and make an appointment via the HealthPartners Appointment Center: 952-967-6857. Please specify the appointment is for a "sports physical".

Students will need to bring the 2 forms the day of the appointment.

Student vaccine records will be able to be accessed through MIIC. The only vaccines students will potentially be due for are the TDap and MMR, which will be available onsite.

Below is an outline of how the sports clinic will work:

Sports Physicals Clinic 

• Students will need to register and make an appointment via the HealthPartners Appointment Center:  952-967-6857
• Students will need to bring 2 forms the day of the event: the MSHSL form and the consent form.
• Student vaccine records will be able to be accessed through MIIC (Minnesota Immunization Information Connection). The only vaccines students will potentially be due for are TDap and MMR, which will be available onsite.

This summer, Twin Cities Orthopedics is once again offering FREE high school sports physicals at 8 different locations.  As a partner school, we're able to give our parents and athletes advanced notice, so you'll be ready to sign up when registration opens on June 21st.   Remember, high school sports physicals are required, and athletes in 7th-12th grade need one every 3 years.
